In response to COVID-19 concerns, we are requesting all patients check-in and complete all paperwork prior to their arrival at our facility. We hope this will shorten your visit and decrease interaction you will have with our clerical staff.
In addition, during confirmation of your appointment we ask you to provide your credit card information so we will be able to charge any co-pays or deductibles, thereby eliminating your need to check-out.
